Rc. No. 147/B1/2011-18,- Smt. O. Manmadha, Staff Nurse, RIMS, Hospital, Ongole,

Prakasam District applied leave on medical grounds from 24-01-2009 to 28-02-2011 and later

proceeded on unauthorized absence. The Director, RIMS, Hospital, Ongole, Prakasam District

has surrendered the services of above Staff Nurse to Regional Director of Medical and Health

Services, Guntur vide ref. 1^st cited.

This Office issued a memo sent to individual to her residential address which was given

previously vide ref. 2^nd cited. The letter return back to Regional Director of Medical and

Health Services, Guntur, due to the postal authorities endorsed that the above addressee is not in

village and says “where abouts not known” dated : 02-05-2011.

The individual submitted a representation by registered post with fitness for duty certificate

an dated : 08-01-2014 in that requested the Regional Director of Medical and Health Services,

Guntur to permit me to join duty after expiry of leave on private affairs from 24-01-2009 onwards

vide ref. 3^rd cited.

The individual not reported before the Regional Director of Medical and Health Services,

Guntur personally. She attended before the enquiry Officer at District Medical and Health

Officer, Ongole, Prakasam District on 12-04-2016. She personally appeared before the

Regional Director of Medical and Health Services, Guntur on 06-10-2016 and submitted a

representation and requested to issue posting orders on humanitarian grounds.

The Director of Public Health and Family Welfare, Andhra Pradesh, Gollapudi,

Vijayawada has issued instructions to intiate disciplinary action against Smt. O. Manmadha,

Staff Nurse, RIMS, Hospital, Ongole, Prakasam District for her unauthorized absence and

conclude it as per existing rules vide ref 4^th and 12^th cited. As for that this office framed the

charges and issued charge memo to individual and appointed the enquiry Officer to conduct

enquiry. Basing on the enquiry report this office issued Show Cause Notice to Smt. O. Manmadha, Staff

Nurse, (U/A) why displinary action / major punishment should not be initiated against

her as per existing rules in force, that her explanation should reach this Office within one Month,

failing which, it will be constructed that she has no explanatin to offer this Office intiate displinary

action/major punishment as per existing rules in force vide ref. 13^th cited. Smt. O. Manmadha,

Staff Nurse, (U/A) submitted the explanation vide ref. 14^th cited.

The Regional Director of Medical and Health Services, Guntur, Zone-III after

careful examination of the matter, has found that Smt. O. Manmadha, formerly Staff Nurse

RIMS, Hospital, Ongole, Prakasam District who proceeded on leave from 24-01-2009 to

28-02-2011 and later proceeded on unauthorized absence from duty submitted explanation to

Show Cause Notice is not convincing.

Therefore Smt. O. Manmadha, formerly Staff Nurse RIMS, Hospital, Ongole,

Prakasam District shall be deemed to have been terminated from service Under Rule 18-A of Andhra

Pradesh Fundamental Rules 5-B of Andhra Pradesh Leave Rules, 1993 issued G.O.Ms.No. 128 &

129 Fin (FR-I) Department dated 01-06-2007 respectively read with G.O.Ms.No. 127 G.A.

(Ser.C) Department, dated : 15-09-2017.
