ORDER :

Rc.No.0632 / E6.B (VC) / 2015,- It has been brought to the notice of

the undersigned through the reference 3 ^rd cited, that Dr.P. Bhanu Tej was

proceeded on Un-authorised absence from duty from 22-02-2014 on wards

while working as Civil Assistant Surgeon, at PMC. Musunuru, Krishna District

without prior permission from the competent authority.

In the reference 4^th read above, Show Cause Notice has been issued

to the said Charged Officer i.e., Dr. P. Bhanu Tej, formerly Civil Assistant

Surgeon, PHC. Musunuru. Krishna District as well as publishing the Show Cause

Notice in the News paper to submit his explanation within (15) days from

the date of publication of the Show Cause Notice as to why the penalty shall

not be imposed on him with effect from the date of his un-authorised absence.

So far no explanation has been received to the Charge Memo either from

the said Civil Assistant Surgeon concerned or from the District Medical and

Health Officer, Krishna, Machilipatnam in this regard.

Therefore, after careful examination of the matter, Dr.P. Bhanu Tej, formerly Civil

Assistant Surgeon, PHC. Musunuru, Krishna District is hereby removed from

service w.e.f. 22-02-2014 i.e., from the date of his unauthorised absence from

duty in terms of G.O.Ms.No. 128., Finance (DR. 1) Department, Dated: 01-06-2007

and G.O.Ms.No. 128. Finance (DR.1) Department, Dated : 01-06-2007.
