ANNEXURE-I

Statement of Articles of Charge framed against Smt.T. Annapurna, Staff Nurse,

formerly of PP Unit. Govt. Hospital. Gudiwada, Krishna Dist., and presently Staff Nurse,

K.M.G.Hospital, Guraja, Krishna Dist.,

***

ARTICLE-I:-

That the said Smt.T.Annapurna, Staff Nurse, while functioning

as Staff Nurse at PP Unit, Govt. Hospital, Gudiwada, Krishna Dist.,

during the period has initially applied for grant of Casual Leave for 4

days from 20.12.05 to 23.12.05 on 19.12.05 on private affairs. Instead

of joining duty on 24.12.05, she extended leave on private affairs

from time to time upto 10.07.2007. Though she was physically and

mentally capable of attending to her duties, she willfully absented from

duty which amounts to her misconduct. Hence the charge.

BASIS OF THE CHARGE:-

The Medicai Officer, PP Unit, Govt. Hospital, Gudiwada, Krishna Dist.,

has reported that Smt.T.Annapurna, Staff Nurse; PP Unit, Govt.

Hospital, Gudiwada, Krishna Dist., has applied Casual Leave for 4

days from 20.12.05 to 23.12.05 and extended leave on private affairs

from time to time ie.from 20.12.05 to 28.02.06, 01.03.06 to 30.04.06,

01.05.06 to 31.10.06, 01.11.06 to 31.01.07, 01.02.07 to 30.04.2007

and from 01.05.07 ro 10.07.07 without showing the specific reasons

under which she proceeded on leave. Hence willful absence from duty

arises as she is physically and mentally capable of attending to her

duties. She has not turned up for duty. Such willful absence amounts

4

ANDHRA PRADESH GAZETTE, July 4, 2018

also to misconduct and from a good and sufficient reason within the

meaning of rule 9 of the CCA Rules, 1991. She must get her leave

sanctioned in the first instance and then proceed. Even with regard to

extension of leave, she must apply fairly in advance before the expiry
