Rc.No. 2263/R2A/2006,- Smt. Y. Lakshmi Kumari, Staff Nurse, Govt. General

Hospital, Vijayawada is on unauthorised absence from duty w.e.f. 11.07.2006.

Articles of charge were issued by this office on 13.09.2010. But they were returned

unserved by the postal authorities on the ground that her whereabouts are not known.

Articles of charge were published in A.P. Gazette on 22.05.2019. Even then she has

not responded.

NOTIFICATIONS BY HEADS OF DEPARTMENTS Etc.,

The Additional District Medical and Health Officer (A&L), Vijayawada has

been appointed as Enquiry Officer to conduct exparte enquiry and submit report.

Accordingly she conducted enquiry on 05.09.2019 and submitted report that the

said Smt. Y. Lakshmi Kumari, Staff Nurse was on unauthorised absence from duty

w.e.f. 11.07.2006 till 05.09.2019 (date of enquiry).

After careful examination of the enquiry report, the Regional Director of

Medical and Health Services, Rajamahendravaram has decided to accept the findings

of the inquiry officer and taken a provisional decision to impose a major punishment of

removal from service on the charge held proved against Smt. Y.Lakshmi Kimari,

Staff Nurse under clause (ix) of Rule 9 of APCS (CC&A) Rules 1991.

Smt. Y. Lakshmi Kumari, Staff Nurse (on unauthorised absence from duty) is

therefore directed to explain why a punishment of removal from service should not be

imposed on her within (15) days from the date of publication of this show cause notice

in A.P. Gazette.

A copy of the enquiry report is enclosed. (4 pages)

If no reply is received within the stipulated time, it will be construed that she

has no explanation to offer and further action will be taken based on the material

available.
