ORDER:

In the reference cited, the District Medical and Health Officer, W.G.District

forwarded the proposals in respect of Dr. Sheik Ayisha, CAS, PHC, Vinjaram, W.G.Dist,

for acceptance of the resignation of the individual, as she was selected in P.G. course

in M.D (Radiodiagnosis) at N.R.I Medical College, Chinakakani, Guntur District

tendered resignation to the post of CAS.

After careful examination of the matter, and keeping in view of the provisions under

Rule 30 of State and Subordinate Services Rules 1996, the Director of Pubiic Health and

Family Welfare, A.P., Vijayawada is pleased to accept the resignation tendered by

Dr.Sheik Ayisha, CAS, PHC, Vinjaram, W.G.Dist, for the post of CAS under the control

of DPH&FW AP from the date of relief from the date of resignation i.e., w.e.f. 28-10-2020.

The District Medical and Health Officer, West Godavari District is requested to

take necessary further action for relief from the present place of working.
