Puheday Patar to Poheday Patar by swearing an affidavit in the Court of Executive Magistrate at

Morigaon on dated 22nd April, 2026.

Henceforth, I shall be known as Poheday Patar instead of Puheday Patar for all purposes.

I, Padumi Das wife of Dharani Das inhabitant of Village- Borgaon in the District of Kamrup have

changed my husband's name from Dharani Das to Dharanidhar Das by swearing an affidavit in the Court of
