I, Kopal Singh W/o Sh. Prateek Chaudhary R/o Tower 9/3C, Bellevue Tower, Central Park 2, Gurugram,

Haryana, inform that I have changed my name from Kopal Singh to Kopal Chaudhary for all future purposes.

In future I will be known as Kopal Chaudhary.
