CHANGE OF NAME

I, Seema alias Chhima Devi wife of Sh. Mayank, Resident of House No. 668, Sector-7, Urban Estate,

Kurukshetra, District Kurukshetra, Haryana, have changed my name from Seema alias Chhima Devi to Chhima Devi.

In future she will be known as Chhima Devi only.
