undersigned under section 13 (3) of Birth and Death Registration Act, 1969 alongwith affidavits
and other documents that his daughter Upasana Devi born on 15-3-2006 at Village Dhardu, Tehsil
Kasauli, District Solan, Himachal Pradesh but her date of birth could not be registered by the
applicant in the birth record of Gram Panchayat's Patta Bariyan, Tehsil Kasauli, District Solan,
Himachal Pradesh.
