Tenders received after the prescribed date and time

will not be accepted. The maximum period required for

delivery of the equipment should also be mentioned.

Tender forms can be obtained from the office of the

undersigned on payment of cost by cash/money order

from 25-6-2007. An amount of Rs. 50 (Rupees Fifty

only) is to be paid extra for tender form by post.

Earnest money deposit of 1% of the total cost will have

to be produced in the form of Demand Draft taken in

favour of the Director of Insurance Medical Services

along with the filled tender form.

Tenders will be opened on the prescribed date in

the presence of the tenderer at the appointed date and

time.

The equipment tendered should confirm ISI

The undersigned authority reserves the right to

accept or reject any tender without assigning any
