Both covers (A & B) must be effectively sealed and put in another cover, which is also sealed and submitted. Cover

A & B will be opened as per the schedule given below. Cover B of those tenderers who satisfy the above conditions in

Cover A will only be opened. The cover B of rejected tenderers will be returned without opening. Details of medicines

and other items to be supplied can be had from the tender form or Govt. of Kerala Website-www.kerala.gov.in. Intending

SPECIAL CONDITIONS

1.

All items should have minimum 75% shelf life left under normal storage condition at the time of supply.

2.

The rates quoted should be inclusive of all taxes.

3.

The Security Deposit furnished will be released only after satisfactory completion of the contract.

4.

Packing and containers of drugs shall be as prescribed in pharmacopeias or Drugs and Cosmetic

Rules 1945. Containers made of good Quality plastics must be used for those items which are to be supplied in

plastic containers.

5.

The rates should be for delivery F.O.R. at 14 district headquarters.

6.

Ex-go down rates and F.O.R. station of despatch rates offered by the firms are not acceptable and such tenders

will be summarily rejected.

7.

The tenderers should give the patent name against each product specifically in their tender. Composition of each

product should be given. Change in the specification of medicine, unit of packing etc. will not be accepted after

placing supply orders.

8.

On the label of each packing it should be inscribed “For the use of Animal Husbandry Department, Kerala”.

9.

The quantities specified in the schedule are only approximate and they may vary according to actual need.

10.

Successful tenderers should supply the items at the approved rates and conditions to the local self Governments

and other Departmental Institutions as per their request.

11.

The rates should be quoted in Indian Currency and payment will be made in Indian Currency only.

12.

The successful tenderers should produce analytical report of every batch of each medicine they have supplied, to

the Controlling Officer of the destination, at the time of the supply and send a copy to the Director of Animal

Husbandry.

13.

The Director of Animal Husbandry reserves all rights to delete any items from the tender without assigning any

reason thereof.

14.

The Director of Animal Husbandry on his authority has the right to inspect the manufacturing unit.

15.

The only jurisdiction for any dispute regarding this purchase will be at Thiruvananthapuram.

The photocopies of the documents produced along with the tender should be attested by a Gazetted Officer.

17.

The medicines are to be supplied at the district headquarters as specified in the supply order and the supply

should be done in a single lot.

18.

Sealed competitive tenders are invited for the

supply of Auto Sampler for Atomic Absorption

Spectrophotometer–1 No. to the Govt. Ayurveda College

Hospital, Thiruvananthapuram for the year 2008-09.

Estimate Cost—Rs. 4,00,000.

Tenders should be superscribed “Supply of Auto

Sampler for Atomic Absorption Spectrophotometer”

addressed to the Principal, Govt. Ayurveda College,

Thiruvananthapuram. The last date of receipt of tenders

will be at 3 p. m. on 11-2-2009. The late tenders will not

be accepted. The tenders will be opened at 3.30 p. m. on

11-2-2009 in the presence of such of the tenderers of their

authorized representatives who may be present at that time.

Indending tenderers may on application to the Principal,

Government Ayurveda College, Thiruvananthapuram to

obtain tender forms on which tenders should be

submitted. Application for the tender forms should be

accompanied by cash remittance of Rs. 900 which is the

price fixed for a form and which is not refundable under

any circumstances. Tender forms are not transferable.

Sale of tender forms will be closed at 1 p. m. on 11-2-2009.

Cheque, Postal Order etc. will not be accepted towards the

cost of forms. Those who wish to purchase tender forms

by post shall send an additional amount of Rs.50 by

Money Order to cover postal charges. Duplicate tender

forms if required will be issued at Rs. 450 per copy.

Intending tenderers should deposit an amount of 1%

of the total cost quoted rate as Earnest Money Deposit by

Demand Draft in favour of the Principal, Government

Ayurveda College, Thiruvananthapuram together with an

agreement in Kerala Stamp Paper worth Rs. 50. The rate

quoted shall remain firm for acceptance till 31-7-2009. The

Principal, Ayurveda College, Thiruvananthapuram will have

powers to postpone/cancel the tender.

approved/authorized Distributor for the use of ESI

Hospital, Ezhukone and Feroke.

Anaesthesia Machine— 2 Nos. (with 2 Vaporizers

The firm should submit the following certificates

not been and are not being supplied to any

other organization at a rate lower than being

quoted here.

(2) A certificate about satisfactory performance of

the equipment duly authenticated by other

existing users of the equipment.

(3) Firms should also give a certificate that spares of

the equipment quoted will be made available

at least for five years.

(4) Firm should undertake to enter into Annual

Maintenance Contract atleast for five years after

completion of warranty and accordingly quote the

rates for Annual Maintenance Contract in the

tender.

The firm has to submit two bids, technical bid and

price bid each on the same day and but in separate

covers. The technical bid will be opened at first instance

and decisions of Technical Evaluation Committee will be

accepted regarding conformity with given specification.

As regards price bid eligible tender i.e. who qualifies the

Technical Evaluation Committee evaluation at first instance

will be opened. If any firm quotes more than one model

(i.e. higher and lower model). EMD of 1% separate for

both quoted items should be submitted according to value

of quoted item. In case only one EMD is followed/

enclosed for two models quoted, EMD will be treated as

for the lowest one.

The number and date of opening of the tender should

be superscribed on the envelope containing the tender.

Acceptance and rejection of tender will be governed by

Stores Purchase Rules.

Tender forms can be obtained either direct from the

office of the Director of Insurance Medical Services on

payment of its actual cost by cash/money order from

12-1-2009 or on post on request by paying Rs. 50

(Rupees Fifty only) additionally over and above the cost

of tender forms. Filled tender forms not subscribed to

Stores Purchase Rules and above conditions will be

rejected summarily.

Tenders will be opened as specified above as

enjoined in the Store Purchase Rules.

The equipment tendered should confirm ISI

specification. The prices quoted should be inclusive of

all taxes, duties and transportation charges to the place of

delivery/specified by Purchaser.

Notwithstanding the above, the undersigned reserves

the right to accept or reject any/all tenders without

assigning any reason thereof but subject to Store

Purchase Rules. The purchase will be made strictly

invoking Stores Purchase Rules as applicable to any other

purchase under Government Department and any

omissions/oversight crept in the tender shall be treated as

void if the clause/rules in Store Purchase Rules commit the

same.
