all exisiting terms and conditions in all Govt. tenders.

by the Chalan receipt of the remittance of the cost of

tender form + VAT. The cost of tender form should be

remitted in Treasury under the Head of Account

“0210-Medical & Public Health-80-800-88-Other Receipts”

and VAT (ST+ AST) under the Head of Account “0040-

Sales Tax-102- State Sales Tax-01-Tax”. Cost of tender form

will not be returned at any cost.

Tenders should be accompanied by an EMD of 1%

of the amount by Demand Draft in favour of District

Medical Officer of Health, Palakkad from any Nationalised

Bank and also a preliminary agreement in a stamp paper

worth ` 100. Late tenders will not be entertained. The

tender should contain the rates quoted for each item with

specifications if any.

The tenders received within the prescribed time will

be opened at 3.30 p. m. on 2-2-2011 in the presence of

such tenderers or their authorised representatives who

may be present at that time. The undersigned reserves the

right to accept or reject any tenders without assigning

even if any change in the dealership of the product

supplied. The tender sholuld be submitted the

specification of the product with broacher of brand name.
