TENDER NOTICE

No. B2/18441/2011.

16th November 2011.

Sealed competitive and superscribed tenders are

invited from reputed Pharmaceutical manufactures/

Suppliers of insecticides for control of Communicable

Diseases of the Kannur District.

Tender should be superscribed “Tenders for the

supply of insecticides for the Control of Communicable

Diseases” for the year 2011-12 and addressed to the

District Medical Officer of Health, Kannur, Civil Station P. O.,

Kannur-670 002. The item-wise rates quoted should be

including Sales Tax, VAT etc.

Intending tenderers may an application to the District

Medical Officer, (Health) Kannur, obtain the requisite tender

forms against cash remittance, in which the tender to be

submitted Cheques, Postal Orders, Demand Draft etc. will not

be accepted towards the cost of tender forms and the tender

forms will not be sent by V. P. P.

List of Insecticides and condition of tenders can be

had from the office along with tender forms during the

working hours.

Probable Amount of Cost—` 1,00,000.

Cost of tender form :

Original—` 400+12.5% VAT.

Duplicate—` 200+12.5% VAT.

Last date and time of sale of tender forms—

19-12-2011 to 22-12-2011.

Last date and time of receipt of tender forms—

23-12-2011, 5 p. m.

Date and time for opening of tender forms—

28-12-2011, 11 a. m.

Date up to which the rates are to be remain firm for

acceptance—31-3-2012.

Period of supply—Within 8 days of receipt of

supply order.

Further details if any can be had from the office of

the undersigned during working hours.
