also known as T. K. C. Aneefa in the Birth Certificate with

Registration No. 2075/04 dated 28-5-2004 of my minor

daughter M. Fathima issued by the Registrar of Births and

Deaths, Payyanur Municipality and also known as

Haneefa Thekke Koleyath in the Indian Passport

No. E. 8649184 dated 1-4-2004 issued by the Passport

Officer, Kozhikode is one and the same person. Hereafter I

will be known by the name Haneefa Thekke Koleyath

only.

This change will come into effect in all records

related to me.
