Sulaika. P. V., in the Birth Certificate with Registration
No. 224/2010, date of Registration 19-3-2010, issued by
the Registrar of Births and Deaths Kadavallur Grama
Panchayat, of my another Minor son Shayan. K. T., and
as Bushara P., in the School Admission Register Extract
with Admission No. 3561 date of Admission 4-6-2012, of
Ideal English School, Kadakasseri, of my minor son
Shamil. K. T. is one and the same person. Hereafer I will
be known by the name Bushara. P. V. only.
This change will come into effect in all records
related to me.
