Birth Certificate with Registration No. 808/1992, date of
Registration 23-5-1992, issued by the Registrar of Births
and Deaths, Kasaragod Municipality, of my daughter
K.S. Aminath Saira, known as Naseera in the Electoral
Identity Card No. KL/0l/00l/210305, dated 18-2-1999, issued
by the Electoral Registration Officer, Manjeshwar
Constituency, All India Secondary School Examination,
2009 Certificate S. No. SSE/2009/0293164 with Roll
No. 4164261 of March 2009, issued from the Central Board
of Secondary Education, Delhi, of my daughter
K.S. Aminath Saira and known as Ayishath Naseera
Kambar in the Passport No. J 1884047, dated 25-5-2010,
issued by the Assistant Passport Officer, Kozhikode, is
one and the same person. Hereafter I will be known by
the name Ayshath Naseera only.
This change will come into effect in all records
related to me.
