Register No. 461277 of March 2001, also known as
Surya Nuchilakathu Puthukudi in the Passport
No. L2846988 issued on 26-9-2013 by the Assistant
Passport Office, Kozhikode, Marriage Certificate
No. 10/2013 dated 23-9-2013 issued by the Marriage
Officer, Kuthuparamba is one and the same person.
I have embraced Islam Religion from Hindu Thiyya
Community as per Discharge Certificate No. 5198 dated
14-1-2014 issued from Therbiyathul Islam Sabha,
Mukhadar, Calicut, with a new name Sarah. Hereafter
I will be member of Islam Religion and have changed my
name as Sarah Fahad consequent on my marriage.
This change will come into effect in all records
