as T. K. Sarabi, in the Election Identity Card No. JWB
1876770 dated 27-3-2003, issued by the Electoral
Registration Officer, Trikaripur, LA Constituency, as
Sarabi. T. K. in the S.S.L.C. No. P 038097 with
Register No. 117002 of March 2014, of my son
Shiyas Muhammed. T. K., Ration Card No. 2469048887
(Sl. No. 1) dated 24-12-2008 issued by the Taluk Supply
Officer, Hosdurg is one and the same person. Hereafter, I
will be known as by the name Sarabi. T. K. only.
This change will come into effect in all records

Muhammed Habeeb Manzil, XVII-502A, Elambachi P. O.,
South Trikaripur, Hosdurg Taluk, Kasaragod District,
holder of S.S.L.C. No. EE. 27563 with Register No.
401479 of August 2011, have embraced Islam Religion
from Hindu Vedar Community as per Discharge Certificate
No. 1923 dated 21-12-2002 and renewed on 23-3-2014
issued from Tharbiyathul Islam Sabha Mukhadar Calicut
with a new name Saifudheen. Hereafter I will be a member
of Islam Religion and will be known by the name
Saifudheen and will sign accordingly.
This change will come into effect in all records
related to me.
