"Supply
of Physiotherapy Equipments for the Koumarabrithya
Department of Government Ayurveda College,
Thiruvananthapuram” addressed to the Principal, Govt.
Ayurveda College, Thiruvananthapuram. The last date of
the receipt of tenders will be at 3 p. m. on 30-11-2015. The
late tenders will not be accepted. The tenders will be
opened at 3.30 p. m. on 30-11-2015 in the presence of such
of the tenderers or their authorized representatives who
may be present at that time.
Intending tenderers may on application to
the Principal, Government Ayurveda
College,
Thiruvananthapuram to obtain tender forms on which
tenders should be submitted. Application for the tender
forms should be accompanied by cash remittance of
` 788 (Cost of tender form ` 750 + VAT ` 38) which is
the price fixed for a form and which is not refundable
under any circumstances. Tender forms are not
transferable. Sale of tender forms will be closed at 3 p. m.
on 29-11-2015. Cheque, Postal Orders etc. will not be
accepted towards the cost of forms. Those who wish to
purchase tender forms by post shall send an additional
amount of ` 50 by money order to cover postal charges.
Duplicate tender forms, if required will be issued at ` 394
per copy.
Intending tenderers should submit an amount of 1%
of the quoted amount as Earnest Money Deposit by
Demand Draft in favour of the Principal, Government
Ayurveda College, Thiruvananthapuram together with an
agreement in Kerala Stamp Paper worth ` 200. The rate
quoted shall remain firm for acceptance till one year from
the date of acceptance. The Principal, Govt. Ayurveda
College, Thiruvananthapuram will have powers to
postpone/cancel the tender. If Government have declared
30-11-2015 as a public holiday, the tender will be opened at
the same time on next working day. Further details can be

The envelops containing the tender should be
superscribed “Tender for the Supply of Lab Chemicals
under the scheme 2202-03-103-76 Development of Lab &
Library-other charges (Plan) 2015-16” and addressed to
The Principal, Government College, Kottayam,
Nattakom P. O., Kottayam-686 013. Last date for the
receipt of tender is 2 p. m. on 30th November 2015. Late
tenders wi1l not be accepted. The tenders will be
opened at 3 p. m. on 30th November 2015 in the
presence of such tenderers or their authorized
representatives who may be present at that time.
Intending tenderers may on application to
The Principal, Government College, Nattakom P. O.,
Kottayam obtain the requisite tender forms on which
tenders should be submitted. Application for tender form
should be accompanied by a cash remittance of ` 420
(including tax) which is the price fixed for a form and
which is not refundable under any circumstances. Demand
Draft for ` 420 on the local branch of S. B. T. drawn in
favour of The Principal, Government College, Kottayam
and self addressed stamped cover should be enclosed
with the application for tender forms to be sent by post.
The tender forms are not transferable. Sale of tender forms
will be closed at 1 p. m. on 30th November 2015.
Chequs Postage Stamps etc. will not be accepted towards
the cost of tender forms nor will the forms be sent per
V. P. P. Duplicate tender forms, if required will be issued at
` 210 (inclusive of taxes) per copy.
The rate quoted should be valid upto 31-3-2015. The
articles should be supplied within 30 days on receipt of
the supply order, in concerned Department at your
expense. Rates, tax and condition of supply of articles
should clearly be stated separately in respect of each of
them. An agreement in the prescribed form in Kerala
Government Stamp Paper worth ` 200 should be enclosed
with the tender.
The tender should be accompanied by an EMD of
1% (one percent) of the total cost of the articles tendered
for, either in cash or crossed Bank Draft or TSB deposits
or Govenment pronote of Bank guarantee or NSL. Tenders
without sufficient EMD will be rejected. EMD will carry no
interest. Firms who are exempted from EMD should furnish
attested copy of exemption orders. More details can be

the Supply of Items/Laboratory Equipments under
“Starting of new courses-2015-16” and addressed to the
Principal, SNGS College, Pattambi, Mele Pattambi P. O.,
Pin-679 306. Last date for the receipt of tender is 2 p. m.
on 28-11-2015. Late tenders will not be accepted. The
tenders will be opened at 3 p. m. on the same day in the
presence of such tenderers or their authorized
representatives who may be present at that time.
Intending tenderers may on application to the
Principal, SNGS College, Pattambi P. O., Mele Pattambi,
Pin-679 306 to obtain the requisite tender form on which
tenders should be submitted. Application for the tender
form should be accompanied by a cash remittance of
` 1,050 (` 1,000 + VAT) which is the price fixed for form/
set of forms and which is not refundable under any
circumstances. ` 50 extra for sending forms by post. The

CONDITIONS
1.
Supply of Polythene covers should be completed
before 15-12-2015.
2.
The receipt should be obtained from concerned
Range Officer, Social Forestry Unit, after the
completion of the supply of Polythene covers as
per agreement.
3.
If the supply is not completed within the dates
specified in item No. (1) above, the tender will be
cancelled and action taken to purchase the items at
the risk and loss of the original bidder.
4.
The Fixed Deposit towards EMD should be
pledged in the name of the Asst. Conservator of
Forests, Social Forestry Division, Alappuzha payable
at State Bank of India, Civil Station Branch,
Alappuzha and should be accompanied along with
the filled up tender forms.
5.
SSI units are exempted from remitting EMD; such
units should produce necessary eligiblity
Certificates obtained from the Manager, District
Industries Centre concerned.
6.
Rate to be quoted per Kg for supplying Polythene
bags should include transporting charge and
existing taxes.
7.
2 Polythene covers should be produced along with
the tender application as samples.
8.
The successful tenderer should furnished 5%
amount of the PAC in addition to the EMD as
security deposit for the contract; and enter into
an agreement with the Forest Department within
7 days from the date of intimation regarding the
acceptance of the tender.
9.
On satisfactory completion of supply and all the
above conditions, the security deposit will be
released to the tenderer on production of necessary
tax clearence certificate.
10.
The tenderer is liable to follow Kerala Store
Purchase Rules.
11.
All the conditions stipulated as per Kerala Forest
Code and Kerala Store Purchase Manual is
applicable for this tender also.

SPECIAL CONDITIONS
(a)
The successful tenderer should have the responsibility of obtaining the Licence for the required VHF network
from the Wireless and Coordination Wing (WPC), Ministry of Communication and Information Technology,
Government of India, New Delhi in the name of Kerala Forest Department. All the formalities for procuring the
Licence and remitting of Licence fee & Royalty charges for all equipment for one year should be fulfilled by the
tenderer and handed over the required documents to the authority before commissioning of the system. Payment
will be released only after obtaining licence for the required VHF network from the Wireless and Coordination
Wing (WPC), Ministry of Communication and Information Technology, Government of India, New Delhi.
(b) The Bidder should be a reputed Original Equipment Manufacturers (OEM) or authorized dealer of OEM, who is
having an Authorization Certificate from the OEM to participate in the tender and Certificate from the OEM for
sales and service.
(c)
The Bidder must fulfill the following minimum qualifying criteria to prove the technocommercial competence and
submit the documents in support thereof :
• Valid Authorization letter from OEM to attend this particular tender. Certificate of Registration/
Incorporation Certificate of Bidder. Valid VAT/Sales Tax Registration Certificate, Valid Income Tax
Registration Certificate, Valid Service Tax Registration Certificate should produce along with the tender.
• The bidder should have positive net worth for each of the last three Financial Years ending on
31-3-2015. Bidder’s P&L and Balance Sheet/audit report of last 3 years should be submitted along with
tender document.
• The Tenderer/Manufacturer shall have valid Dealer Possession License (DPL) from WPC, Ministry of
Communication and Information Technology, Government of India, New Delhi to fulfill all WPC
regulations. Documentary proof should be provided.
• The bidder (authorized dealer/distributor) should have the dealership/distributorship for atleast 5 (five)
continuous consecutive years with single OEM.

• The bidder/manufacturer shall have a minimum of 5 years experience in the Manufacture/supply of same
OEM products like VHF/VHF Handsets, Base stations, Repeaters & related accessories for Kerala Excise/
Police/Forest/any Government Department. The documentary proof should be provided.
• All Radios Quoted must conform to MIL STD 810 C, D as per US Military specifications.
(d) The tenderer/OEM should have an authorized service center for the offered equipment in Kerala for rendering
after sales service with qualified engineers and service personnel’s. Documentary proof should be provided.
(e) The offered equipment should be a part of approved equipment list of WPC. Documentary proof should be
provided.
(f)
The bidder should submit Product data sheets for all the products quoted with detailed technical specifications.
(g) For items in the schedule of equipments, payment will be made to the actual quantity of materials used.
(h) Unit rates for equipment, wiring labour and condition work shall remain valid till the completion of work in all
respects and shall be free from variations due to increase or decrease in the cost of materials, labour or any
other reason whatever. Unit rates shall become applicable to variations in quantities due to changes
modifications in the installation.
(i)
The contractor shall guarantee that all equipments shall be free from any defect due to the defective materials
and bad workmanship and that the equipment shall operate satisfactorily and that the performance and
efficiencies of the equipment shall not be less than the guaranteed values.
(j)
The successful bidder has to supply, install and commission, operationalize the system and ensure that all the
stated requirements are met during the acceptance before it is handed over to the Department. Warranty on all
the hardware components should be valid for a period of atleast 1 year from the date of acceptance of the
equipment.
(k) During the warranty period, the tenderer shall be fully responsible for the manufacturer’s warranty in respect of
proper design, quality and workmanship of all the equipment, accessories, etc. and the tenderer shall attend to
all the hardware problems on site and shall replace the defective parts at no extra cost to ensure the
uninterrupted office functioning.
(l)
After the warranty period, the tenderer shall execute a Non Comprehensive AMC for the equipment and the
amount quoted for the AMC for 5 years after warranty period of one year shall be considered for the evaluation
of this tender.
(m) During the Non Comprehensive AMC Period, the tenderer should be fully responsible to attend the hardware
problems on site and shall replace the defective parts at the actual/market rate of the material. Failure of it will
charge a penalty towards the contractor and the authority will fix the amount at moment in time.
(n) During the erection and at the guarantee period, the contractor shall associate with the department staff to
familiarize with the operation and maintenance of the system. If required by the Department, the contractor shall
also agree to train members of the department, free of cost, either at his works or at such place or places as may
be considered suitable by the department.

The tenders should be superscribed “Tender for the supply of Wireless base station devices and addressed to

There may be differences in quantities of Schedule I
and II according to actual requirement at the time of
execution of work.
Tender forms shall be issued up to 1 p. m. on
1-12-2015 at the Office of the Assistant Conservator of
Forests, Social Forestry, Olavakkode, Palakkad. Tender
forms duly filled up and sealed shall be accepted on the
same day up to 3 p. m. and tenders will be opened in the
presence of tenderers present at that time at
3.30 p. m. If the date of tender happens to be a public
holiday the same will be held on the next working day at
the same time and venue.
Conditions
(1)
Supply of polythene cover and seeds should be
completed from the date of entering into the
agreement.
(2)
The receipt should be obtained from the
concerned Range Officers of Social Forestry
Range after completion of the supply.
(3)
If the supply of seeds/polythene cover are not
completed within the days specified the tender
will be cancelled and purchase shall be made at
the risk and loss of the original bidder.
(4)
Final payment of the seeds shall be made only
after ensuring germination of seeds and final
amount will be paid after deducting the
proportionate amount. 25% of each preliminary
bill will be deducted as retention and the same
will be released subject to verification of
percentage of germination.
(5)
EMD should be remitted as demand draft drawn
on State Bank of India, Civil Station Branch
pledged in the name of Assistant Conservator of
Forests, Social Forestry Division, Palakkad and
accompanied along with filled up tender forms.
(6)
The quantities mentioned in the Schedules are
approximate and may vary according to actual
requirements.
(7)
Rates quoted for supply of polythene bags and
seeds should include transportation charges.
(8)
Two polythene covers should be produced as
sample along with tender application for
polythene covers.
(9)
The successful tenderer should remit 5% of the
contract amount as security deposit for the
contract in addition to EMD and enter into an
agreement with Forest Department within 7 days
from the date of intimation regarding the
acceptance of tender.
(10)
On satisfactory completion of supply and all the
above conditions the security deposit will be
released to the tenderer on production of
necessary tax clearance certificate.
(11)
The tender is bound by the conditions issued by
Government of Kerala regarding Purchase of
Stores.
(12)
All the conditions stipulated as per Kerala
Forest Code and Kerala Store Purchase manual
shall be applicable for this tender also.

Instruments as per Tender Notice No. B2-15698/15 dated
29-10-2015” and should be addressed to the District
Medical Officer (Health), Kannur.
Last date for receipt of tenders will be on 28-11-2015
at 3 p. m. Late tenders will not be accepted. The tender
will be opened at 3 p. m. on 30-11-2015 in the presence of
such of the tenderers or their authorized representatives
who may be present at that time.
Indending tenderers may on application to the
District Medical Office (Health), Kannur obtain the
requisite tender forms on which tender should be
submitted. Application for the tender form should be
accompanied by a cash remittance of ` 600 + VAT, which
is the price fixed for a tender form and which is not

TENDER SCHEDULE
1.
The period for which the rates to be in firm—
One year.
2.
The rates should be inclusive of all taxes, otherwise
tender will be rejected without any notice and for
free delivery at the department concerned.

Advance payments conditions will not be considered.

Tender forms are neither transferable nor the cost of
it will be refundable under any circumstances.
5.
Tenders are to be sent with details and illustrated
pamphlet (wherever necessary) neatly typed.
Manuscripts should be avoided as far as possible.

Supply of Two Digital Levels by the undersigned for
and on behalf of the Governor of Kerala.
Description—Supply of Digital level.

Detailed specification is available with the office on
all working days and along with the tender form.
The equipment should be guaranteed for one year
from the date of supply. A security deposit of 5% shall
be retained with Government for one year and the entire
maintenance shall be attended by the accepted tenderer(s)
free of cost during the above period.
The equipment should be delivered within thirty
days after receiving the order at the place specified by the
Chief Hydrographer, Hydrographic Survey Wing,
Thiruvananthapuram.
The contractor should enclose documents with full
details pertaining to the supply of the same to any
Government/Semi-Government Organizations/Public Sector
Undertakings along with the tender form.
The special conditions pertaining to the tender are
available with the tender documents. The technical and
the financial bids are to be submitted separately and the
processing of the tenders will be based on this. The firm
period will be six months from the date of opening of the
tender.
Tenders should be superscribed “Tender for the
supply of Two Digital Levels” with the No. and date and
must be addressed to the Chief Hydrographer,
Hydrographic Survey Wing, Thiruvananthapuram-695 001,
so as to reach on or before 28-11-2015 upto 1 p. m.
Late tenders will not be accepted. The tenders will be
opened at 3 p. m. on the same day in the presence of
such tenderers or their representatives who are present at
that time.
The tenderer should remit Earnest Money Deposit in
the Government Treasury under the Head of Account
“8443-00-103-SD Receipts” in favour of the Chief
Hydrographer, Hydrographic Survey Wing payable at
Thiruvananthapuram and the Chalan remitted should
produce along with the tender.
Intending tenderers may obtain the requisite
tender form on application to the undersigned.
Application for the tender form should be accompanied
by the cash remitted chalan under the Head of Account
“1051-80-800-96-III-MISC-HSW” through e-Treasury
payment for the cost of tender form (DD/Cheques not
acceptable). Sale of tender form will be closed at 12 Noon
on 28-11-2015. The tender forms are not transferable.
Those who want to get the tender form by post should
also enclose a self-addressed envelop bearing postage
stamp of ` 100 well in advance.
The tenderers should enclose a preliminary agreement
in Kerala Stamp Paper worth ` 500 and the required
EMD for each tender. The tender without preliminary
agreement and EMD will be rejected outright. The final
acceptance of the tender rests entirely with the
Government, who reserves the right to accept or reject any
