Panchayat, of my son Mathew Shaji, known as
Shaji Mathew in the Aadhaar No. 4997 4045 5783,
issued from Unique Identification Authority of India, and
known as Shaji in the Electoral Identity Card No.
KL/01/005/297144, dated 2-1-1999, issued by the
Electoral Registration Officer, Trikaripur Constituency, is
one and the same person. Hereafter I will be known by
the name Shaji Mathew only.
This change will come into effect in all records
related to me.
