NOTIFICATION

F. No.24/5/2022-B&D(MCCD):- In exercise of powers conferred by sub-section (2) of

section 10 of the Registration of Births & Deaths Act, 1969 (18 of 1969), the Administrator

of the Union Territory of Lakshadweep, having regard to the facilities available in the areas

secified hereunder, hereby requires the medical practitioner who attended the deceased

during the last illness to issue Medical Certificate as to the cause of death (in the specified

Form 4 or 4 A) in the following locality/health facilities with effect from the date of publication

of this natification namely:-

(III) All hospitals including Nursing and Maternity Homes whether managed by public

or private organizations and societies of urban/rural area(s) in the district of

entire Union Territory of Lakshadweep.

and

(IV) All private medical practitioners of urban and rural areas of the District

of Lakshadweep in the Union Territory of Lakshadweeep

These institutions shall present the Medical Certificate as to the cause of Death

in Form 4 to the concerned Registrar of Births & Deaths in their jurisdiction at the time of

giving information of death as required under the said act.

In case of domiciliary events, the private medical practitioner or doctor who

attended last illness shall provide Medical Certificate as to the cause of death to the relative

of the deceased in Form 4 A to furnish it to the concerned Registrar of Births and Deaths at

the time of giving information of death.
