3. In Secondary School Leaving Certificate and Birth Certificate, my daughter’s

name is correctly recorded as NAJMA FARSANA M. K. However, it has come to

my attention that in her passport reflects her name as NAJMA FARSANA.

4. I hereby affirm and declare that my daughter’s true and original name is NAJMA

FARSANA M. K, and any variation from this in any official records is incorrect.
