AFFIDAVIT

I CHERIYAKOYA. K S/o Muthukoya, Aged 69 years, residing at Kuttilammada (H),

Agathi Island, Lakshadweep do hereby solemnly affirm and state as follows.

1.

I am the native of Agathi Island, Lakshadweep and residing at Kuttilammada (H),

Agathi Island Lakshadweep.

2.

The passport authority of India was issued a passport bearing No. J5244800 on

27/05/2011 and in the said passport, my name was entered as CHERIYAKOYA HAJI.

Whereas, other documents pertains to me shows may name as CHERIYAKOYA. K and

Cheriyakoya Kuttilammada. It is solemnly affirmed that initial K denotes after name is a

short of my house name Kuttilammada.

I swear this affidavit to state that Cheriyakoya Haji, Cheriyakoya. K and Cheriyakoya

Kuttilammada are one and the same person.

All the facts stated above are true to the best of my knowledge and belief
