on 13/01/2012 and in the said passport my name was entered as PURAKKARA SYED

MOHAMMED KOYA. Whereas, other documents such as matriculation certificate and

service-related documents pertains to me shows my name as SYED MUHAMMEDKOYA. P.

it is solemnly affirmed and state that my correct name and address appear as

SYED MUHAMMEDKOYA. P
