AFFIDAVIT

I, SAYED KOYA, S/o Muthukoya, aged 64 years, residing at Pattakal, Amini Island,

Union Territory of Lakshadweep, do hereby solemnly affirm and state as follows:

1. That I am the deponent herein and I am well aware of the facts of this affidavit.

2. That I am a holder of an Indian Passport bearing number K7254211, Wherein

my name is mentioned as SYED KOYA, and my date of birth is recorded as

01/07/1961.

3. That I also hold an Aadhaar Card bearing number 3574 3657 4049, in which

my name is recorded as SAYED KOYA, and my date of birth is mentioned

as 29/06/1961.

4. That due to inadvertence and clerical error, there is a discrepancy in my

name and date of birth as recorded in my Passport and Aadhaar Card.

5. That both the names SYED KOYA and SAYED KOYA, and both the dates of

birth 01/07/1961 and 29/06/1961, refer to one and the same person, i. e.,

myself.

6. That I am making this affidavit to declare and confirm that my correct name and

date of birth, which I wish to be officially recognized, is SAYED KOYA and

29/06/1961.

concealed therein.
