AFFIDAVIT

I, UMMUL KULUS PANDARAPURA, D/o Kader Uriyoda, aged 54 years, residing at

Pandarapura, Agatti Island, Union Territory of Lakshadweep, do hereby solemnly affirm and

state as follows:

1. That I am the deponent herein and I am well aware of the facts of this affidavit.

2. That I am a holder of an Indian Passport bearing number K3880390, wherein

my name is mentioned as UMMA KULUZ PANDARAPPURA.

3. That I also hold an Aadhaar Card bearing number 9084 2182 8850, in which

my name is recorded as UMMUL KULUS PANDARAPURA.

4. That due to inadvertence and clerical error, there is a discrepancy in my

name as recorded in my Passport and Aadhaar Card.

5. That both the names UMMUL KULUS PANDARAPURA and UMMA

KULUZ

PANDARAPPURA, refer and pertains to one and the same

person, i. e., myself.

6. That I am making this affidavit to declare and confirm that my correct name,

which I wish to be officially recognized, is UMMUL KULUS PANDARAPURA.

7. That I undertake to use my correct name in all my future records and

documents.

8. That I am executing this affidavit to be submitted to the concerned

authorities for the purpose of rectification and publication in the

Gazette, as required.
