I, ABDUL HAKEEM AP S/o Mohammed Puthiyaveedu, aged 49 years, residing at

Alachappada, Kavaratti Island, Lakshadweep Pin-682555 do hereby solemnly affirm and state as

follows.

That I am married to Raziya Fathima Manzil, Fahima Manzil Kavaratti Island, Lakshadweep.

out of the wedlock, we have four children namely (1) Mohammed Mufeed (2) Mufeeda Hakh

(3) MunnavviraHakh (4) Mohammed Munavvir.

It is found that name of my daughter MUNNAVIRA HAKH FM is wrongly appearing in the

Adhaar as Munavvara Hakeem FM. Due to the error in the name printed on Adhaar, it is difficult to

enrol the details of my daughter to the Apaar Id in school. It is necessary to update Adhaar with true

name and address to enable enrolment in Apaar Id.

I hereby affirm and state that my original name is MUNNAVIRA HAKH FM and I do not wish

to call and appear as Munavvara Hakeem FM.

This affidavit is sworn for the purpose of notification in the Lakshadweep Gazetteer stating

that the Correct name of my daughter is MUNNAVIRA HAKH FM for updating of Adhaar card and

other records.

I, SAYED MOHAMMED SHIHABUDDIN, S/o K.M. Koya, aged 43 years, residing at

Shekkiriyammada House, Mecheri, Andrott Island, Union Territory of Lakshadweep, do hereby

solemnly affirm and state as follows.

That I am the deponent herein, and I am well aware of the facts stated in this Affidavit

That I am a holder of an Indian Passport bearing number WO901054, in which my name

is recorded as SAYED MOHAMMAD SHIHABUDDIN S. In the said passport, my house name is

recorded as SHAKERIYANMMADA.

That I also hold an Aadhaar Card bearing number 9379 4308 8169, in which my name

is recorded as SAYED MOHAMMED SHIHABUDDIN S, and my house name is recorded as

SHEKKIRIYAMMADA.

That due to inadvertence and clerical error, there is a discrepancy in my name and house

name as recorded in my Passport and Aadhaar Card.

That both the names SAYED MOHAMMED SHIHABUDDIN S and SAYED MOHAMMAD

SHIHABUDDIN S refer to one and the same person, ie., myself.

Likewise, both house names SHEKKIRIYAMMADA and SHAKERIYANMMADA refer to one

and the same house.

That it is also noted in my Passport that my name appears as SAYED MOHAMMAD

SHIHABUDDIN S, where “S” is an abbreviation of my house name, which has already been enered

as my surname.

That I am marking this affidavit to declare and confirm that my correct name and house

name, which I wish to be officially recognized, are SAYED MOHAMMED SHIHABUDDIN and

SHEKKIRIYAMMADA, respectively.

That I undertake to use my correct name in all future records and documents.

That I am executing this affidavit to submit to the concerned authorities for the purpose of

rectification and publication in the Gazette, as required.
