SHAFANA KAMAR G B.

5. That due to inadvertence and clerical error, there is a discrepancy in her name as

recorded in the Passport and other records such as Aadhaar Card and Certi昀椀cate.

6. That both the names SHAFANA KAMAR G B and SHAFANA KAMAR refer to one

and the same person, i.e., my daughter.

7. That I am making this a昀케davit to declare and con昀椀rm that my daughter’s correct

name, which I wish to be o昀케cially recognized, is SHAFANA KAMAR G B.

8. That I undertake to use my daughter’s correct name as SHAFANA KAMAR G B in all

my future records and documents.

9. That I am executing this a昀케davit to be submitted to the concerned authorities for the

purpose of recti昀椀cation of my daughter’s name in her passport and for publication in
