ORDER UNDER SECTION 30 OF POLICE ACT-1861
Whereas the undersigned is of the opinion that the situation in all the Islands in Union
Territory of Lakshadweep is such that assemblies and procession and usage of loud speakers
in public places are required to be conducted under license and whereas in the judgment
of the District Magistrate, Lakshadweep such assemblies/processions and usage of loud
speakers would, if uncontrolled, result in breach of peace.
I, Utkarsha IPS, Superintendent of Police, Union Territory of Lakshadweep now prescribe
that such license under section 30 of Police Act is necessary for maintaining peace and law
& order and for smooth progress of assemblies/processions and usage of loud speakers in
public places at Kavaratti, Androth, Minicoy, Amini, Kalpeni, Kadmath, Kiltan, Chetlat, Agatti
and Bitra.
The promoters intending to conduct any assembly/procession and usage of loud speakers
shall apply for such license at least 48 hours prior to the event before undersigned with a
copy to the Officer-In-Charge of the concerned Police Station and abide by the conditions
prescribed in the license.
However, public function of purely cultural nature and functions organized by various
Government Department are exempted from the ambit of this order.

AFFIDAVIT FOR CHANGE OF NAME
I, Saleem Ahmed Saleem, son of Kunhikoya Maidanachetta, residing at Biriyakkal
House, Chetlat Island, Union Territory of Lakshadweep do hereby solemnly affirm and
state as follows:
That my name as per my existing records is Saleem Ahmed Saleem.
That I have changed my name from Saleem Ahmed Saleem to Saleem Ahmed for all
purposes in the future.
That henceforth I shall be known and called as Saleem Ahmed in all my records, documents,
and for all official purposes.
That this affidavit is made for the purpose of evidence of my change of name.
I declare that the above statements are true to the best of my knowledge and belief.

other laws in force.
The order shall come into operation with immediate effect and shall remain effective for

AFFIDAVIT
I, MUTHUBI CHEKKIRIMMADA D/o Moosan Kunnipura, aged 69 years, residing at
Chekkirimmada (H), Chetlat Island, Lakshadweep do hereby solemnly affirm and state as
follows.
1. I am a native of Chetlat Island, Lakshadweep and residing at Chekkirimmada (H),
Chetlat Island, Lakshadweep.
2. The Passport Authority of India was issued a passport bearing No. M6536077
on 23/02/2015 and in the said passport, my name was entered as MUTHUBI
CHEKKIRIYAMMADA . Whereas, other documents pertains to me shows my
name as MUTHUBI CHEKKIRIMMADA. It is solemnly affirmed that my actual
name may be correctly spelled as MUTHUBI CHEKKIRIMMADA.
3. I swear this affidavit to state that MUTHUBI CHEKKIRIMMADA and MUTHUBI
CHEKKIRIYAMMADA are one and the same person.
All the facts stated above are true to the best of my knowledge and belief.
