follows.
1. I have born on 10/04/2006 and my birth place and native is Kiltan Island, Lakshadweep.
2. It is found that my name is appearing as MOHAMMED ALI HASSAN SHADULI
NORTH BANGLOW in the passport issued by the passport authority of India
whereas my true and correct name appearing in other records pertains to as
MOHAMMED ALI HASSAN SHADULI.
3. I hereby affirm and state that my original name is MOHAMMED ALI HASSAN
SHADULI and I do not wish to call and appear as MOHAMMED ALI HASSAN
SHADULI NORTH BANGLOW.
4. I am decided to keep my name as MOHAMMED ALI HASSAN SHADULI in the
entire records and documents related to me.
5. It is difficult to travel with passport and with my identity card as name is appearing
differently in both. Hence, a uniform appearance in the records pertains to me is
