I, Mohammed Akheel Mubarak D.A, S/o. Mohammed Kasim P.C, and Thajunnisa K.V,
residing at Darul Arham House, Amini Island, U.T. of Lakshadweep-682552, aged 27 years,
do hereby solemnly affirm and state as follows:-
1. That I was born on 06/07/1998 (as per Birth Certificate) and my birth place and native
place is Amini Island, Lakshadweep.
2. That my name has been recorded as Darul Arham Mohammed Akheel Mubarak in the
following documents:
a) Passport bearing No. S5036851 dated 30.07.2018, issued from Kochi.
b) Continuous Discharge Certificate (CDC) No. MUM 430755 dated 24.05.2021.
c) Seafarer’s Certificate.
3. That my name has been recorded as Mohammed Akheel Mubarak D.A in the following
academic and identification records:
a) SSLC Book bearing Roll No. 4253255 (Sl.No.SSE/2014) dated 19.05.2014, issued by
the Principal, Kendriya Vidyalaya, Kavaratti (CBSE).
b) Plus Two (+2) Certificate bearing Roll No. 4832763 (Sl. No. SSCE/2016/1219587)
dated 21.05.2016, issued by the Controller of Examination, CBSE.
c) Degree Certificate Reg.No.56216011 dated 21.08.2020, issued by the Controller of
Examination, Cochin University of Science and Technology (CUSAT), Kochi.
d) Cast Certificate vide F. No. 13/4/2021-SDO (A)/47 dated 16.06.2021, issued by the
Executive Magistrate, Amini Island, U.T. of Lakshadweep.
e) Aadhaar Card bearing No. 6388 5322 5892, issued by the Government of India.

4. That my name has been recorded as Mohammed Akheel Mubarak in the Birth Certificate
dated 24.07.1998, issued by the Health Supervisor (Registrar of Births and Deaths),
Corporation of Cochin.
I do hereby declare that all the above names- (“Darul Arham Mohammed Akheel Mubarak”,
“Mohammed Akheel Mubarak D.A”, and “Mohammed Akheel Mubarak”- refer to one
and the same person, that is myself.
I, Mohammed Akheel Mubarak D.A, the above-named deponent, do hereby verify that the
contents of this affidavit are true and correct to the best of my knowledge and belief, and

2. It is found that my name is appearing as Sulaikha in the passport issued by the
passport authority of India and whereas my true and correct name appearing in
other records pertains to me as SULAIKHA P.N
