AFFIDAVIT FOR NAME CHANGE
I, Abdul Khader T P, S/o Koyamma Koya A., aged 44 years, residing at Thattampokkada
House, Androth Island, U.T of Lakshadweep, Pin - 682551 do hereby solemnly affirm and
state as follows:-
1. That I am the deponent herein and I am well aware of the facts of this affidavit.
2. That my daughter is minor and she is a holder of an Indian Passport bearing number
X6483024, wherein her name is mentioned as FATHIMA SANA instead of FATHIMA
SANA A.
3. That she also hold a Certificate issued from the office of the Registrar of Birth and Death,
Androth, in which her name is recorded as FATHIMA SANA.
4. That she also hold Certificate of the Central Board of Secondary Education bearing
Registration number T124/77004/0045, in which her name is recorded as FATHIMA
SANA A.
5. That due to inadvertence and clerical error, there is a discrepancy in my daughter’s name
as recorded in the Passport and other records such as Birth Certificate and Certificate of
the Central Board of Secondary Education.
6. That both the names FATHIMA SANA A and FATHIMA SANA refer to one and the same
person, i.e., my daughter.
7. That I am making this affidavit to declare and confirm that my daughter’s correct name,
which she wish to be officially recognized, is FATHIMA SANA A.
8. That I under take to use my daughter’s correct name as FATHIMA SANA A in all my future
records and documents.
9. That I am executing this affidavit to be submitted to the concerned authorities for the
purpose of rectification of my daughter’s name in passport and for publication in the
Gazette, as required.
I solemnly affirm and declare that the statements made above are true and correct to the
best of my knowledge and belief, and that nothing material has been concealed therein.

on Adhaar, it is difficult to enrol the details in other official requirements,

appearance in the official records pertains to me.

By this deed I DINESH SHANTARAM now lately called DINESH CHAHDRA SHANTARAM
