5. That due to inadvertence and clerical error, there is a discrepancy in my name as recorded

in the Passport.

and the same person, i.e., myself.

7. That I am making this affidavit to declare and confirm that my correct name which I wish

to be officially recognized, is SAYEED SHAIK KOYA.

address is “AYISHOMMA, BANDER, AGATTI ISLAND LAKSHADWEEP.

2. It has come to my notice and found that my name is erroneously spelled in some

of the records, Aadhaar card and documents pertains to me that “AISHOMMA

BANDER” instead of printing and recording it as “AYISHOMMA”.

3. It is necessary to correct my name as AYISHOMMA, in the entire records related to

me for uniform appearance and hence, a notification in the Government gazetteer is

highly necessary.

I swear this Affidavit for the purpose of changing my name and address in the entire

records, testimonials, Adhaar Card etc. related to me.

2. That by virtue of this affidavit, I changed my name from Raliyath Roulommechetta

to Kadeeshabi Chettathada and hence forth I shall be known as the Kadeeshabi

Chettathada for all purposes.

name is mentioned as SUBAIDA MUTHUKOYA instead of SUBAIDA K C.
