एखाद्याा व्यक्तीची ओळख पटवि�ण्यासाठीी बहुुवि�ध दस्तऐवज सादर करण्याची गरज टाळून लाभाार्थ्यां�ंɕनाा सोईस्कर व अखंडिि’त रीतीने त्यां�चेे

हक्क, थेेट प्राप्त होणे शक्य होतेे ;

आणि� ज्याअर्थीी, सार्ववजनि�क आरोग्य वि�भाागाद्वाारे (यात यापुढेे ज्याचाा नि�र्देश, “वि�भााग” असा केला आहे) प्रशाासन केल्या जाणााऱ्या

आणि� राज्य आरोग्य हमीी सोसायटीद्वाारे (यात यापुढेे ज्याचाा नि�र्देश, “एजन्सी” असा केला आहे) अंमलबजावणीी केल्या जाणााऱ्या रोकड

वि�रहि�त (कॅशलेेस) आरोग्य सेवेसाठीी एकात्मि��क महाात्माा ज्योति�रााव फुले जन आरोग्य योजनाा (एमजेेपीजेएवााय) आणि� आयुुष्माान भाारत-

प्रधाानमंंत्रीी जन आरोग्य योजनाा (एबी-पीएमजेेएवााय) (यात यापुढेे ज्याचाा नि�र्देश “योजनाा” असा केलेला आहे) महााराष्ट्र राज्यात (यात

यापुढेे ज्याचाा नि�र्देश, “राज्य” असा केला आहे) राबवि�ण्यात येत आहे ;

आणि� ज्याअर्थीी, या योजनेमध्ये, महााराष्ट्र राज्याच्या एकत्रि�त नि�धीीमधूून केलेल्या आवर्ती खचाा�चाा समाावेश होतो ;

त्याअर्थीी, आता, आधाार (वि�त्तीीय व इतर अर्थथसहााय्य, लाभ व सेवाा यां�चेे लक्ष्यििȤित वि�तरण) अधि�नि�यम, २०१६ (२०१६ चाा १८)

(यात यापुढेे ज्याचाा नि�र्देश, “उक्त अधि�नि�यम” असा केला आहे) याच्या कलम ७ च्या तरतुुदींनुुसाार, महााराष्ट्र शाासन, याद्वाारे, पुढीीलप्रमााणे

अधि�सूूचि�त करीीत आहे :—

१. (१) या योजनेखालील लाभ मि�ळण्यासाठीी इच्छुुक असणााऱ्या पात्र लाभाार्थ्या�ा�नेे, याद्वाारे, आधाार क्रमांं�क धाारण केल्याचाा

पुराावाा किं�ंवाा आधाार क्रमांं�काचे अधि�प्रमााणन केल्याचाा पुराावाा सादर करणे आवश्यक असेल.

(२) या योजनेखालील लाभ मि�ळण्यासाठीी इच्छुुक असणाारा जो लाभाार्थीी, आधाार क्रमांं�क धाारण करीीत नााही अथवाा ज्याने

अद्यााप आधाार क्रमांं�क मि�ळण्यासाठीी नााव नोंंदणीी केलेली नााही, अशाा कोणत्याही लाभाार्थ्या�ा�नेे, त्याच्या आई-वडिि’लांं�च्या किं�ंवाा

पालकांं�च्या संंमतीीच्या अधीन राहून (लाभाार्थीी बालक असल्यास), आधाार क्रमांं�काच्या नााव नोंंदणीीसाठीी अर्जज करणे आवश्यक

असेल, परंतुु, तो, उक्त अधि�नि�यमााच्या कलम ३ नुुसाार आधाार क्रमांं�क मि�ळण्यासाठीी हक्कदाार असावाा, आणि� असा बालक, आधाार

क्रमांं�क नााव नोंंदणीी करण्यासाठीी कोणत्याही आधाार नााव नोंंदणीी केंद्रााला [ भाारतीय वि�शि�ष्ट ओळख प्राधि�करण (युुआयडीएआय)

संंकेतस्थळ www.uidai.gov.in या संंकेतस्थळाावर सूची उपलब्ध आहे ] भेट देईल.

(३) आधाार (नााव नोंंदणीी व अद्ययाावत करणे) वि�नि�यम, २०१६ याच्या वि�नि�यम १२ नुुसाार, वि�भाागाने, त्याच्या

अंमलबजावणीी अभि�करणाामाार्फत, ज्यां�नीी अद्यााप आधाारसाठीी नााव नोंंदणीी केलेली नााही अशाा लाभाार्थ्यां�ंɕकरि�ताा आधाार नााव नोंंदणीी

सुुवि�धाा देऊ करणे आवश्यक असून जर, संंबंधि�त गटात किं�ंवाा तालुुक्यात किं�ंवाा तहसि�ला

ात कोणतेेही आधाार नााव नोंंदणीी केंद्र

स्थि�ित नसेेल तर, वि�भााग, त्याच्या अंमलबजावणीी अभि�करणाामाार्फत, युुआयडीएआयच्या वि�द्यमाान नि�बंंधकााच्या समन्वयााने किं�ंवाा

तेे स्वतःःच युुआयडीएआयचे नि�बंंधक होऊन सोयीच्या ठि�कााणी आधाार नााव नोंंदणीी सुुवि�धाा पुुरवील :

परंतुु, व्यक्तीस, आधाार क्रमांं�क नेमून देण्याच्या वेळेपर्यंंत, पुढीील दस्तऐवज सादर करण्याच्या अधीन राहून, अशाा व्यक्तीस,

या योजनेखालील लाभ देण्यात येतील :—

एकः अठराा वर्षेे वयााखालील मुुलां�करि�ताा :—

(क) जर पाच वर्षेे वय पूर्णण झााल्यानंंतर, बालकाचे नााव नोंंदवि�ण्यात आले असेल (बायोमेट्रि�क संंकलनाासह)

तर, त्याची आधाार नााव नोंंदणीी ओळख पावतीी, किं�ंवाा बायोमेट्रि�क अद्ययाावत ओळख पावतीी; आणि�

(ख) पुढीील दस्तऐवजांं�पैकी कोणतााही एक दस्तऐवज :—

(एक) जन्म दाखला; किं�ंवाा समुुचि�त

प्राधि�करणााने दि—लेला जन्मााचाा अभि�लेेख; किं�ंवाा

(दोन) माातापि�त्याचे नााव अंतर्भू�त असलेेले, शााळेच्या मुुख्याध्यापकाने यथोोचि�तरि�त्या स्वााक्षरी केलेले

शााळेचे ओळखपत्र ; आणि�

(ग) वि�द्यमाान योजनेच्या माार्गगदर्शशक तत्त्वांं�नुुसाार माातापि�त्याशी किं�ंवाा कायदेशीर पालकांं�शी असलेेल्या

लाभाार्थ्यां�ंɕच्या नाातेेसंंबंधााचाा पुराावाा म्हणूून पुढीीलपैकी कोणतााही एक दस्तऐवज :—

(एक) जन्म दाखला; समुुचि�त

प्राधि�करणााने दि—लेला जन्मााचाा अभि�लेेख; किं�ंवाा

(दोन) शि�धाापत्रि�काा ; किं�ंवाा

(तीन) मााजी-सैनि�क अंशदाायी आरोग्य योजनाा, (ईसीएचएस) पत्रि�काा; किं�ंवाा राज्य कामगाार वि�माा

महाामंडळ (इएसआयसी) पत्रि�काा ; किं�ंवाा केंद्र सरकार आरोग्य योजनाा (सीजीएचएस) पत्रि�काा ; किं�ंवाा

(चाार) नि�वृृत्तीीवेतन पत्रि�काा ; किं�ंवाा

(पाच) सेेनाा कँन्टि�िन पत्रि�काा ; किं�ंवाा

(सहाा) कोोणतेेही सरकारी कुुटुंंȾब हक्कपत्र; किं�ंवाा

(सात) वि�भाागाने वि�नि�र्दिदि—िष्ट केल्याप्रमााणे इतर कोणतााही दस्तऐवज:

दोनः अठराा वर्षांंɕवरीील लाभाार्थ्यां�ंɕकरि�ताा :—

(क) जर नााव नोंंदणीी केली असेल तर, ति�चीी आधाार नााव नोंंदणीी ओळख पावतीी; आणि�

(ख) पुढीीलपैकी कोणतााही एक दस्तऐवज :—

(एक) बँकेची किं�ंवाा डाक कार्या�लयाची छायाचि�त्र असलेेली खातेे-पुस्ति�िकाा; किं�ंवाा

(दोन) स्थाायी लेखा क्रमांं�क पत्र ( PAN card); किं�ंवाा

(तीन) पारपत्र (पासपोोटट); किं�ंवाा

(चाार) शि�धाा वााटप पत्रि�काा; किं�ंवाा

(पाच) मतदाार ओळखपत्र; किं�ंवाा

(सहाा) मनरेेगा पत्रि�काा; किं�ंवाा

(सात) छायाचि�त्र असलेेली कि�साान खातेे - पुस्ति�िकाा; किं�ंवाा

(आठ) मोटार वााहन अधि�नि�यम, १९८८ ( १९८८ चाा ५९ ) अन्वयेे लायसन देणााऱ्या प्राधि�करणााने

दि—लेले वााहन चाालन लायसन; किं�ंवाा

(नऊ) राजपत्रि�त अधि�कााऱ्याने किं�ंवाा तहसि�लदाराने शाासकीीय नााममुुद्रि�त पत्रावर दि—लेले अशाा व्यक्तीचे

छायाचि�त्र असलेेले ओळख प्रमााणपत्र; किं�ंवाा

(दहाा ) वि�भाागाने वि�नि�र्दिदि—िष्ट केल्याप्रमााणे इतर कोणतााही दस्तऐवज:

परंतुु आणखीी असे की, त्या प्रयोजनाार्थथ एजन्सीने वि�शेेषरि�त्या पदनि�र्देशि�त केलेल्या अधि�कााऱ्याद्वाारे वरीील

दस्तऐवज तपाासण्यात येतील.

२. या योजनेअंतर्गत, लाभाार्थ्यां�ंɕनाा सुुलभतेेने लाभ पुुरवि�ण्याच्या दृष्टीने, आधाारच्या उक्त आवश्यकतेेची लाभाार्थ्यां�ंɕनाा

जाणीव करून देण्याकरि�ताा मााध्यमांं�माार्फत व्यापक प्रसि�द्धीी देण्यात येईल याची सुुनि�श्चि�िती

ी करण्यासाठीी वि�भााग, अंमलबजावणीी

अभि�करणाामाार्फत आवश्यक त्या सर्वव व्यवस्थाा करीील.

३. जेव्हाा लाभाार्थ्यां�ंɕच्या नि�कृृष्ट बायोमेट्रि�क्समुुळेे किं�ंवाा इतर कोणत्याही कारणांं�मुळेे आधाार अधि�प्रमााणन नि�ष्फळ ठरेेल

तेेव्हाा, अशाा सर्वव प्रकरणी, पुढीील सुुधाारात्मक कार्ययतंत्रांं�चाा अंगीकार करण्यात येईल :—

(क) बोटाच्या ठशांं�चाा दर्जा� नि�कृृष्ट असेल अशाा बाबतीत, अधि�प्रमााणनाासाठीी डोळ्यातील बुुब्बुळााची प्रति�माा स्कॅन

करण्याची (आयरि�स स्कॅन) किं�ंवाा चेहऱ्याचे अधि�प्रमााणन करण्याची सुुवि�धाा अंगीकारण्यात येईल, त्याद्वाारे, वि�भााग, त्यांं�च्या

अंमलबजावणीी अभि�करणाामाार्फत, अखंडरीतीने लाभ देण्यासाठीी बोटांं�च्या ठशांं�च्या अधि�प्रमााणनाासह डोळ्यातील बुुब्बुळााची

प्रति�माा स्कॅन करण्यासाठीी (आयरि�स स्कॅनर) किं�ंवाा चेहऱ्याचे अधि�प्रमााणन करण्यासाठीी तरतुुदी करीील;

(ख) बोटाचे ठसेे घेेणे किं�ंवाा डोळ्यातील बुुब्बुळााची प्रति�माा स्कॅन (आयरि�स स्कॅन) करणे किं�ंवाा चेहऱ्याचे अधि�प्रमााणन

करणे यांं�माार्फत बायोमेट्रि�क अधि�प्रमााणन यशस्वी होत नसेेल अशाा बाबतीत, जेव्हाा जेव्हाा सुुसाध्य व अनुुज्ञेेय असेल तेेव्हाा,

आधाार एक वेळ परवलीीचाा शब्द (वन टाइम पासवर्डड), किं�ंवाा यथाास्थि�िति�, मर्या�दि—त काळासाठीी वैधताा असलेेला वेळ आधाारि�त

एक वेळ परवलीीचाा शब्द (वन टाइम पासवर्डड) याद्वाारे अधि�प्रमााणन करण्यात येईल;

(ग) जेव्हाा बायोमेट्रि�क किं�ंवाा आधाार एक वेळ परवलीीचाा शब्द (पासवर्डड) किं�ंवाा वेळ आधाारि�त एक वेळ परवलीीचाा

शब्द (पासवर्डड) शक्य नसेेल तेेव्हाा अशाा इतर सर्वव बाबतीत, योजनेखालील लाभ, प्रत्यक्ष आधाार पत्राच्या आधाारे देता येतील,

त्याची अधि�प्रमााणताा, आधाार पत्रावर मुुद्रि�त केलेल्या शीघ्र प्रति�सााद संंकेतांं�कामाार्फत (क्यूआरकोड) पडतााळता येऊ शकेेल

आणि� वि�भाागाद्वाारे, त्याच्या अंमलबजावणीी अभि�करणाामाार्फत, सोईस्कर ठि�कााणी शीघ्र प्रति�सााद संंकेतांं�क (क्यूआरकोड)

वााचकााची आवश्यक ती व्यवस्थाा करण्यात येईल.

४. यात यावर काहीही अंतर्भू�त असलेे तरीी, कोणत्याही बालकास, अधि�प्रमााणन करून त्याद्वाारे त्याची ओळख सि�द्ध करण्यास

किं�ंवाा आधाार क्रमांं�क धाारण करीीत असल्याचाा पुराावाा देण्यास नि�ष्फळ ठरल्यास, अथवाा नााव नोंंदणीीसाठीी अर्जज करून देखील

ज्याला आधाार क्रमांं�क नेमून दि—लेला नााही अशाा बालकाच्या बाबतीत, योजनेखालील लाभ नााकारण्यात येणाार नााहीत. परि�च्छेद १

च्या उप-परि�च्छेद (३) च्या परंतुुकाच्या खंड एक (ख) व एक (ग) मध्ये नमूूद केल्याप्रमााणे, इतर दस्तऐवजांं�च्या आधाारे त्याची

ओळख पडतााळून त्याद्वाारे त्याला लाभ देण्यात येतील, आणि� जेव्हाा अशाा इतर दस्तऐवजांं�च्या आधाारे लाभ दि—ले जातात तेेव्हाा,

त्या अभि�लेेखाच्या नोंंदी ठेवण्यासाठीी स्वतंंत्र नोंंदवहीी ठेवण्यात येईल, जि�चेे वि�भाागाद्वाारे त्यांं�च्या अंमलबजावणीी अभि�करणाामाार्फत

नि�यतकाालि�क पुनर्वि�िलोकन व लेखापरीक्षण करण्यात येईल.

५. वरीील गोष्टींशि�वााय, कोणतााही खरा लाभाार्थीी, या योजनेखालील त्याच्या उचि�त लाभाापासून वंचि�त राहणाार नााही याची

सुुनि�श्चि�िती

ी करण्याच्या दृष्टीने, राज्य शाासनाामधीील व संंघरााज्य प्रशाासनाातील संंबंधि�त वि�भााग, थेेट लाभ हस्तांं�तरण (डीबीटी)

अभि�याान, कॅबि�नेेट सचि�वाालय, भाारत सरकार, क्रमांं�क. डी-२६०११/०४/२०१७-डीबीटी, दि—नांं�क १९ डिि’सेंंबर, २०१७ च्या

कार्या�लयीन ज्ञापनाात (http://dbtbharat.gov.in/ या संंकेतस्थळाावर उपलब्ध) वि�नि�र्दिदि—िष्ट केल्यानुुसाार अपवााद हााताळणी

यंत्रणेेचे अनुुपाालन करीील.

६. ही अधि�सूूचनाा, शासकीय राजपत्रात प्रसि�द्ध झााल्याच्या दि—नांं�काापासून अंमलाात येईल.

७. मुुख्य कार्ययकारी अधि�काारी, महाात्माा ज्योति�रााव फुले जन आरोग्य योजनाा, राज्य आरोग्य हमीी सोसायटी, मुं�बई याबाबतीत

आवश्यक ती पुढीील कार्ययवााही करतील.

NOTIFICATION

Aadhar (Targeted Delivery of Financial and Other Subsidies, Benefits and Services) Act, 2 016.

No. Aadhar-2023/C.R.258/Health-6.—Whereas, the use of Aadhaar as an identity document of

delivery of services or benefits or subsidies simplifies the Government delivery processes, brings in

transparency and efficiency and enables beneficiaries to get their entitlements directly in a convenient

and seamless manner by obviating the need to produce multiple documents to prove one’s identity;

And whereas, the Integrated Mahatma Jyotirao Phule Jan Arogya Yojana (MJPJAY) and

Ayushman Bharat-Pradhan Mantri Jan Arogya Yojana (AB-PMJAY) (hereinafter referred to as the

scheme) for cashless health service is being implemented in the state of Maharashtra, (hereinafter

referred to as the State) administrated by Public Health Department (hereinafter referred to as

the Department) and implemented by State Health Assurance Society (hereinafter referred to as

Agency) ;

And whereas, the Scheme involves recurring expenditure incurred from the Consolidated Fund

of the Government of Maharashtra.

Now, therefore, in pursuance of section 7 of the Aadhaar (Targeted Delivery of Financial and

Other Subsidies, Benefits and Services) Act, 2016 (18 of 2016) (hereinafter referred to as “the said

Act”), the Government of Maharashtra hereby notifies the following, namely :—

(1) An eligible Beneficiary desirous of availing the Benefit under the Scheme shall

hereby be required to furnish proof of possession of the Aadhaar number or undergo Aadhaar

authentication.

(2) Any beneficiary desirous of availing the Benefit under the Scheme, who does not

possess the Aadhaar number or, has not yet enrolled for Aadhaar, shall be required to make an

application for Aadhaar enrolment subject to the consent of his parents or guardians (in case

of child Beneficiaries), provided that he is entitled to obtain Aadhaar as per section 3 of the

said Act and such child shall visit any Aadhaar enrolment centre (list available at the Unique

Identification Authority of India (UIDAI) website www.uidai.gov.in) to get enrolled for Aadhaar.

(3) As per regulation 12 of the Aadhaar (Enrolment and Update) Regulations, 2016,

the Department through its Implementing Agency, is required to offer Aadhaar enrolment

facilities for the Beneficiaries who are not yet enrolled for Aadhaar and in case there is no

Aadhaar enrolment centre located in the respective Block or Taluka or Tehsil, the Department

through its Implementing Agency shall provide Aadhaar enrolment facilities at convenient

locations in coordination with the existing Registrars of UIDAI or by becoming a UIDAI Registrar

themselves :

Provided that till the time Aadhaar is assigned to the individual, the benefits under the

Scheme shall be given to such individual, subject to production of the following documents,

namely :—

I : For children below eighteen years old,—

(a) if the child has been enrolled after attaining the age of five years (with

biometrics collection), his Aadhaar Enrolment Identification slip, or of bio-metric

update identification slip; and

(b) any one of the following documents, namely :—

(i) Birth Certificate; or Record of birth issued by the appropriate authority; or

(ii) School identity card, duly signed by the Principal of the school, containing

parents’ names; and

with the parent or legal guardian as per the extant Scheme guidelines, namely :—

(i) Birth Certificate; or Record of birth issued by the appropriate authority; or

(ii) Ration Card; or

(iii) Ex-Servicemen Contributory Health Scheme (ECHS) Card; or Employees’

State Insurance Corporation (ESIC) Card; or Central Government Health Scheme

(CGHS) Card; or

(iv) Pension Card; or

(v) Army Canteen Card; or

(vi) any Government Family Entitlement Card; or

(vii) any other document as specified by the Department:

II : For Beneficiaries above eighteen years old

(a) if he has enrolled, his Aadhaar Enrolment Identification slip; and

(b) any one of the following documents, namely :-

(i) Bank or Post office Passbook with Photo; or

(ii) Permanent Account Number (PAN) Card; or

(iii) Passport; or

(iv) Ration Card; or

(v) Voter Identity Card; or

(vi) MGNREGA card; or

(vii) Kisan Photo passbook; or

(viii) Driving license issued by the Licensing Authority under the Motor

Vehicles Act, 1988 (59 of 1988); or

(ix) Certificate of identity having photo of such person issued by a Gazetted

Officer or a Tehsildar on an official letter head; or

(x) any other document as specified by the Department:

Provided further that the above documents may be checked by an officer specifically

designated by the Agency for that purpose.

2. In order to provide benefits to the beneficiaries under the Scheme conveniently, the

Department through its Implementing Agency shall make all the required arrangements to

ensure that wide publicity through the media shall be given to the beneficiaries to make them

aware of the said requirement.

3. In all cases, where Aadhaar authentication fails due to poor biometrics of the

beneficiaries or due to any other reason, the following remedial mechanisms shall be adopted,

namely :—

(a) in case of poor fingerprint quality, Integrated Risk Information System scan or

face authentication facility shall be adopted for authentication, thereby the Department

through its Implementing Agency shall make provisions for Integrated Risk Information

System scanners or face authentication along with finger-print authentication for delivery

of benefits in a seamless manner;

(b) in case the biometric authentication through fingerprints or iris scan or face

authentication is not successful, wherever feasible and admissible authentication by

Aadhaar One Time Password or Time-based One-Time Password with limited time validity,

as the case may be, shall be offered;

One-Time Password authentication is not possible, Benefits under the Scheme may be given

on the basis of physical Aadhaar letter whose authenticity can be verified through the Quick

Response (QR) code printed on the Aadhaar letter and the necessary arrangement of Quick

Response code reader shall be provided at the convenient locations by the Department

through its Implementing Agency.

4. Notwithstanding anything contained herein above, no child shall be denied benefit

under the Scheme in case of failure to establish his identity by undergoing authentication, or

furnishing proof of possession of Aadhaar number, or in the case of a child to whom no Aadhaar

number has been assigned, producing an application for enrolment. The Benefit shall be given

to him by verifying his identity on the basis of other documents as mentioned in clauses I.(b) and

I.(c) of the proviso of sub-paragraph (3) of paragraph 1, and where Benefit is given on the basis

of such other documents, a separate register shall be maintained to record the same, which shall

be reviewed and audited periodically by the Department through its Implementing Agency.

5. In addition to the above, in order to ensure that no bona fide Beneficiary (other than

children) under the Scheme is deprived of his due Benefits, the concerned Department in the

State Governments and Union Territory Administrations shall follow the exception handling

mechanism as specified in the Office Memorandum of Direct Benefit Transfer Mission, Cabinet

Secretariat, Government of India No. D-26011/04/2017-DBT, dated the 19th December 2017

(available on https://dbtbharat.gov.in/).

This notification shall come into effect on the date of its publication in the Official

Gazette.

The Chief Executive Officer, Mahatma Jyotirao Phule Jan Arogya Yojana, State

Assurance Society Mumbai, shall take necessary further action in the matter.
