NOTIFICATION

The 2nd May, 2019

No. 2175–IIEX-23/2019/Ex.— Whereas, it has been reported that at least five

persons of Tihidi/ Chandabali area of Bhadrak District died after consuming liquor along

with other items on 28th-29th April, 2019 and several others have been admitted to

different hospitals for treatment;

And, whereas, the State Government are deeply concerned with the incident and of

the opinion that the same being a definite matter of public importance should be enquired

into by the Revenue Divisional Commissioner (Central Division), Cuttack;

Now, therefore, Revenue Divisional Commissioner (Central Division), Cuttack shall

enquire into and report in respect of the following matters within thirty days from the date

of publication of this Notification in the Odisha Gazette, namely:—

a. The causes of this tragedy;

b. Persons responsible for the same;

c. Lapses if any, on the part of any Excise / Police official;

d. Recommendations regarding actions deemed necessary to prevent such

incident in future;

ORDER

Ordered that the copies of the Notification be published in the Odisha Gazette for

information of general public and three hundred copies thereof be furnished to this

Department, copies thereof be forwarded to all Departments / Principal Accountant

General (A & E), Odisha, Bhubaneswar / all Collectors / all Superintendents of Police /all

Excise Deputy Commissioners / all Superintendents of Excise.
