Address : At/P.O. Kulei, P.S. Nachuni, Dist. Khordha, PIN-752034, have changed my name from

"Bhikari Charan Bhola" to "Purna Chandra Bhola''. Henceforth, I shall be known as Purna Chandra

Bhola for all purposes.
