I, Subhasmita Samal, Vill/P.O. Orasahi, P.S. Chandabali, Dist. Bhadrak, Odisha declare that, I have

changed my name from "Subha Smita Samal" to "Subhasmita Samal''. Henceforth, I shall be

known as Subhasmita Samal for all purposes.
