By virtue of an affidavit sworn before the Executive Magistrate, Jharsuguda on the 31st

January 2024, I, Shri Gadadhar Parida, S/o Late Subal Charan Parida, Resident of Village

At Gitagram, P.O. PTC, District Angul, Odisha-759123, have changed my name from "Sadananda

Parida" to "Gadadhar Parida''. Henceforth, I shall be known as Gadadhar Parida for all purposes.
