P.O. Jagannath Pur-Bachhada, P.S. Basudev Pur, Dist. Bhadrak, Odisha-756046 declare that,

I have changed my name from "Sabita Palei" to "Shubhashree Das''. Hence, I shall be known as

Shubhashree Das for all purposes.
