September 2023, I, Manorama Mallick, W/o Amiya Kumar Mallick, resident of Plot No. 613/1859,

Road-2, Ashutosh Nagar, Kesora, P.S. Saheed Nagar, Bhubaneswar-751002 declare that, I have

changed my name from "Manamohini Mallick" to "Manorama Mallick''. Henceforth, I shall be known

as Manorama Mallick for all future purposes.
