January 2024, I, Pradip Parida, S/o Bishuni Parida, Vill. Harichandanpur, P.O. Khalisahi,

Khandapada, Dist. Nayagarh, Odisha declare that, I have changed my name from "Pradip Kumar

Parida" to "Pradip Parida''. Henceforth, I shall be known as Pradip Parida for all purposes.
