I, P K Sambit Kumar Tripathy, resident at Samagola, P.O./P.S. Kujanga, District Jagatsinghpur,

State Odisha, PIN Code-754141, have changed my name from "Pksambit Kumar Tripatrhy" to

"P K Sambit Kumar Tripathy''. Henceforth, I shall be known as P K Sambit Kumar Tripathy for all

purposes.
