P.O. Abhinab Bidanasi, Cuttack, PIN-753014 declare that, I have changed my name and gender

''Subhashree Behera" (Female) to "Sambhav Behera'' (Male). Henceforth, I shall be known as

Sambhav Behera for all purposes.
