By virtue of an affidavit sworn before the Notary Public, Cuttack on the 9th March 2024,

I, Tapasudha Moharana, W/o Niel Arup Mozumder, At/P.O./P.S. Ghasipura, Dist. Kendujhar, Odisha-

758015, have changed my minor son's name/surname from "Bhaerava Deva Mozumder'' to

"Bhaerava Deva''. Henceforth, he shall be known as Bhaerava Deva for all purposes.
