March 2024, I, Dhuli Behera, son of Dharanidhar Behera, resident of Village Kantilo, P.O. Kuha,

P.S. Airfield, Bhubaneswar, Dist. Khordha, Odisha, PIN-751002, declare that, I have changed my

name from "Nanda Kishor Behera'' to "Dhuli Behera''. Henceforth, I shall be known as Dhuli

Behera for all purposes.
