2023, I, Jagatjita Dash, D/o Narayan Chandra Behera of Village Surubana, P.O. Sarasada,

P.S. Bhandari Pokhari, District Bhadrak, Odisha declare that, I have changed my surname from

"Behera'' to "Dash''. Henceforth, I shall be known as Jagatjita Dash for all purposes.
