By virtue of an affidavit sworn before the Executive Magistrate, Bhubaneswar on the 5th

February 2024, I, Ghasiram Majhi,Vill. Bhalukhudi, P.O. Pataranga, P.S. Korei, Dist. Jajpur, Odisha,

declare that, I have changed my surname from "Ghasiram Murmu'' to "Ghasiram Majhi''. Henceforth,

I shall be known as Ghasiram Majhi for all purposes.
