Via Rasulpur, P.S. Kuakhia, Dist. Jajpur, PIN-755051, Odisha declare that, I have changed my

surname from "Prasanta Kumar Pati" to "Prasanta Kumar Panigrahi''. Henceforth, I shall be known

as Prasanta Kumar Panigrahi for all purposes.
