By virtue of an affidavit sworn before the Executive Magistrate, Panposh, Rourkela on the

1st March 2024, I, Sunita Badaik, W/o Firender Badaik, residing at Qrs. No. D/75, Sector-14,

Rourkela-9, P.S. Sector-15, Dist. Sundargarh, Odisha, have changed my name from "Sunita Hira"

to "Sunita Badaik''. Henceforth, I shall be known, called and identified as Sunita Badaik for all

purposes.
