December 2023, I, Pragnya Priyadarsini Mallick, D/o Debendra Nath Mallick of Vill. Sanilo,

P.O. S. Hazipur, P.S. Tirtol, Dist. Jagatsinghpur, Odisha declare that, I have changed my name

from "Pragnya Priyadardarsini Mallick " to "Pragnya Priyadarsini Mallick''. Henceforth, I shall be

known as Pragnya Priyadarsini Mallick for all purposes.
